Forms and Information

We’re glad you’ve chosen Dr. Cannon and our team to care for your smile, and we’ll do all that we can to make your time with us positive and pleasant. Your first appointment will take about an hour and a half. Please print the pages below, and bring your completed new patient forms with you to your first appointment.

After checking in, you’ll meet with Dora Cain, our hygienist, for a thorough periodontal exam and discussion of what you like and dislike about your smile. She'll answer your questions about homecare and, upon request, show you techniques for effective brushing and flossing. Dora will also take a full set of digital X-rays and ask about your immediate concerns. If you're interested in cosmetic dentistry, feel free to ask her questions. She will be happy to explain teeth whitening, porcelain veneers, bonding, and other procedures that can give you a sensational smile.

Following your consultation with Dora, you’ll meet Dr. Cannon. He’ll review your X-rays and health history before thoroughly examining your teeth, soft tissues, occlusion (how upper and lower teeth fit together), and existing dental work. The doctor will share his findings with you, showing you X-rays and intraoral images of your mouth. If you have questions, please ask. The more you understand about your situation, the better prepared you'll be to make wise treatment decisions. We may also show you CAESY videos, which are short films that describe treatment options. For patients who need extensive work performed, we’ll make models and schedule a 30-minute follow-up consultation to discuss treatment options. Once a plan is decided upon, our front desk will schedule your appointments before you leave the office.
